Tailored Support to Rebuild Lives
At New Journey Supported Living we offer outcome focused, wrap around support to empower individuals to regain their independence and reach their full potential. Our services are holistic, person centred and tailored to meet each individuals’ unique needs.
We understand that people may become homeless for a number of reasons from social circumstances such as a lack of affordable housing, unemployment, and poverty, to being forced into homelessness due to leaving prison, hospital admission, care, the army or escaping violence. For many individuals, life events such as the breakdown of a relationship, losing a job, mental or physical health difficulties, or substance misuse issues put further strain on an already difficult situation and being homeless makes these problems even harder to resolve or move forward. We believe that this can be resolved, and homelessness can be preventable with the right support at the right time.
We provide safe, affordable, and clean accommodation to vulnerable adults who have been identified as having additional support needs, that would otherwise struggle to engage in the community and manage their own tenancies. New Journey Supported Living operate a pathway of independence through offering appropriately staged support to individuals from homelessness, to temporary or emergency accommodation, to supported living when and if required and progression onto independent living.
The support that we offer is person-centred and specific to individual need. We recognise that no individuals’ needs are the same and offer specialised support packages to support and encourage individuals on their journey to living an independent and settled life.
At New Journey Supported Living, we offer a range of specialist housing and additional support that consists of but is not exclusive to the following:
We understand that people become homeless for a range of complex and overlapping reasons. Our temporary / emergency accommodation is a first step onto our pathway and is typically available to individuals who are assessed to have a priority for assistance by local authorities, who are in immediate need of a safe and secure home due to being street homeless, until furthermore long-term housing and support can be offered.
We operate a staged rehabilitative supported living model where we can offer support to individuals that have been released from prison or who are stuck in a cycle of revolving door offending behaviour and homelessness. Here at New Journey Supported Living we offer ‘through the gate’ support as part of our rehabilitative approach as we understand that being released from prison can be very stressful and overwhelming and often, too much for an individual to cope with alone.
We understand that mental health can be both a cause and consequence of homelessness and so we offer specialist accommodation with additional support specific to individual needs. Our tailored and person-centred approach allows us to provide wrap-around support to individuals to find appropriate accommodation, financial support they are entitled to, and engagement with specific health and community care services that they require. This may be through linking in with Prison, Probation and/or community mental health services to try to ensure continuity of care and prevention of individuals ‘slipping through the net’.
We understand that addiction can impact on every area of an individual’s life, and this can often deter services from providing specialist support that individuals really do need. We offer advice, guidance, advocacy on this journey, and support individuals to realise their own potential outside of using substances. We work collaboratively with specialist local substance misuse services to encourage engagement through building realistic steps towards change and building a future towards independence so that individuals can re-gain control of their own life. An individual’s recovery journey is generally never straight forward, and we will support every step of the way.
We understand that appropriate housing is a key component of community health care and positive engagement. We provide a safe and secure home for individuals whose mental health impacts on many aspects of their functioning without support. We understand that everyone’s recovery journey is unique, requiring different levels of support at varying stages. That’s why we offer a broad range of personalised support that is trauma informed and adopts a stair-cased approach, designed to help as individuals’ successful transition from a secure hospital setting to more independent living.
New Journey Supported Living work with local authorities to provide 24hr support to adults with learning disabilities and/or autism. We offer a core of support that is flexible to individual need and provide a modern and comfortable home environment that enables individuals to live as independently as possible.
